Get a library card

Anyone may apply for a free Polk County Public Library card. Please apply in person at either library and bring with you:

1. Proof of identity in the form of a valid picture ID, for example: a Driver’s License or other state-issued ID, School ID, Military ID, Passport, or VA Veteran’s card.

2. Verification of your current mailing address, for example: a utility bill (electric, gas, water, telephone), insurance policy/card, personalized check, bank statement, or tax notice.

Anyone under 16 years of age must be accompanied by a parent/guardian when applying for a library card. The parent/guardian will be responsible for any items checked out by the minor in her/his charge.

An institution, such as a business or private agency (for example, assisted living centers, group homes, or childcare facilities) may also be registered as a library user at no cost.

It is the responsibility of the cardholder to inform the library of any name or address changes.
